I saw the audit exposure building. I decided to do something about it.

As a practicing hospice physician and National Medical Director, I reviewed recertification notes every benefit period. And every benefit period, I saw the same thing — nurses who deeply understood their patients' clinical picture, but who were writing notes that wouldn't survive an audit.

It wasn't a knowledge problem. It wasn't a skills problem. It was a feedback problem. Nurses were documenting in isolation, without any real-time signal about whether their notes met the Medicare LCD criteria that auditors review months later.

"By the time an ADR arrives, the note that triggered it is 90 days old. There's nothing you can do. I wanted to fix the note before it was ever signed."

So I built Hospice Intelligence. Not as a software product, but as a clinical tool — one designed by someone who understands the documentation standards, the audit process, and the pressure hospice nurses work under every day.
